NEW Membership Application Form

Required for HHMFC Membership (checklist)

❑ Copy of MAAC Membership card valid through year of membership application

❑ This fully completed form, signed as applicable (submitting an electronic form is deemed as being signed);

NEW MEMBERS:

– Do not pay now –

Initiation fee* and membership fee is due following your evaluation flight and confirmation that you still wish to join the club.

Membership in the club will be renewable annually, on the 1 st of January.

Fees (to August 1 of the Year):

*Initiation Fee: $50.00 (New adult members only)
Member: $135.00
Junior Member: $30.00 (under 18 as of January 1st)
Social Member: $50.00

After August 1:

New members $70
Junior Member $15
Social Member $25

WAIVER

• I acknowledge and agree that I use the properties and facilities arranged for me by HHMFC at my own risk and I hereby hold harmless HHMFC, its Executive and the owners of any such property and facilities from any suit, cause, action or claim resulting from my use of such property or facility.
• I acknowledge and agree that I am solely responsible for my personal knowledge of, and compliance with any and all applicable Transport Canada regulations.
• I acknowledge and agree that I must hold a current and valid MAAC membership while a club member, and that I must operate in accordance with MAAC codes and guidelines, and am solely responsible for my personal knowledge of, and compliance with any and all applicable MAAC codes and guidelines. (Social Members excepted.)
• I acknowledge and agree that the executive, by majority vote, may rescind the membership of any member for abuse of club facilities or fellow members, and that there is no entitlement to a refund of membership fees.
• I acknowledge and agree that the Federal, Provincial, Regional, or Municipal orders and health recommendations may be in place from time to time, and that I am solely responsible for compliance with them.
• I agree to hold the HHMFC, its officers, directors and instructors blameless in the event of accidents involving damage or loss of models or property, personal injury or loss of life resulting from the operation of any model aircraft, regardless of whether said aircraft was under the control of myself or an instructor at the time of the accident.
• I agree to hold the HHMFC, its officers, directors and instructors blameless in the event of any health-related occurrences that may have occurred as a result of attendance at club property of facilities.
• HHMFC acknowledges that this waiver does not affect the applicant’s right or benefits under any insurance that is provided by the Model Aeronautics Association of Canada (MAAC) or the applicants themselves.
• I hereby apply for membership in the Halton Hills Model Flying Club (HHMFC). I have read and understand the HHMFC Constitution (http://hhmfc.ca/resources) and the MAAC Safety Code (https://www.maac.ca/en/documents.php) and I agree to abide by the rules and regulations as established therein.
